The Canadian Frailty Network (CFN) invites you to submit your innovation for an opportunity to be highlighted at the National Conference on Frailty: Innovation and Collaboration for Impact on September 26-27 at the Delta Marriott Toronto Airport Hotel. Submissions should describe demonstrated or emerging impact on health or social programs, policies or initiatives focused on improving the lives of older Canadians living with frailty and their friend/family caregivers. A select number of innovators will be invited to present their projects in either oral (podium) or storyboard form. This opportunity is open to all public and private innovators implementing practice or policy change in all care settings within Canada.
